In the management of vascular disease, there has been an inexorable drive towards less invasive endovascular treatments. This has substantially altered theattitudes of patients, clinicians and health care providers. Endovascular treatment of carotid stenosis for stroke is no exception. Several trials are running concurrently; these are comparing carotid endarterectomy (CEA) with the less invasive alternative, carotid stenting (CAS). There is already evidence that CAS may be preferable in certain patient populations i.e. those deemed to beat high surgical risk. Furthermore both procedures have comparable benefits in terms of survival free of ipsilateral stroke between three to five years post-procedure. State-of-the-art CAS (with regards use of all available technicalrefinements) is all but five years old. There is considerable interest from clinicians from a variety of clinical backgrounds and from industry in this technique and, therefore, scope for a contemporary practical guide. Illustrated guide to variations in carotid anatomy and relevance to patient selection Practical guide to selection of material, thus optimising outcomes by avoidance of use of inappropriate material Guide to imaging during CAS, thus avoiding errors which may occur through inadequate visualization of anatomy Guide to post procedural care including the management of complications INDICE: How to set up a carotid stenting programme: European and US Perspectives.- First steps: training, support and proctorship.- Pre-procedure imaging.- Patient selection: clinical and anatomical features.- Pharmacological support.- Equipment basic and advanced: an overview.- Procedural imaging: tips andtricks.- Embolic containment: devices and how to avoid disaster.- Dealing with difficult access.- Relevance of periprocedural haemodynamics.- Post procedure care.- Dealing with complications: neurorescue.- Surveillance: critical issues and management of symptomatic restenosis.- Optimizing outcomes.
