It is anticipated that this new book will be sold in conjunction with The Primer on the Rheumatic Diseases, but also that the new book will have major appeal in its own right as the idea represents one that has never been accomplished in the field of rheumatology. Rheumatic diseases pose enormous challenges for most clinicians. Important strides have been made in understanding the pathophysiologic basis of many inflammatory conditions in recent years, but rheumatology remains a discipline in which diagnosis is rooted in the medical historyskillfully extracted from the patient, the careful physical examination, and the discriminating use of laboratory tests and imaging. Moreover, selection ofthe most appropriate therapy for patients with rheumatic diseases also remains heavily reliant upon clinical experience. Medical disciplines such as rheumatology that depend significantly upon clinical wisdom over quantitative diagnostic techniques are prone to the development of systems of ‘Pearls’ and ‘Myths,’ related to the diseases they call their own. For the purposes of this book,a ‘Pearl’ shall be regarded as a nugget of truth about the diagnosis or treatment of a particular disease that has been gained by dint of clinical experience. Conversely, a ‘Myth’ is a commonly held belief that influences the practice of many clinicians – but is false. For a variety of reasons, the field of rheumatology has a long tradition of ‘Pearls’ and ‘Myths,’ and this book will pool together the clinical wisdom of seasoned, expert rheumatologists who participate in the care of patients with autoimmune diseases, systemic inflammatory disorders, and all other rheumatic conditions. Although The Primer presents rheumatology at a level that medical students and house officers understand readily, its mission has never been to provide in-depth clinical wisdom that appeals to experienced general physicians and sub-specialists. This book will capitalize upon the success of The Primer as a way to extend the basic principles elaborated in that textbook, with concrete pieces of clinical know-how that arenot found in traditional texts.
